Crash Reports
The Ohio State Highway Patrol maintains complete crash reports and crash photographs from crashes investigated by the Ohio State Highway Patrol only. The crash reports and photographs are retained for five years.
Please wait a minimum of seven to 10 business days from the date of the crash before attempting to order a crash report and/or photographs to ensure the availability of the completed initial report. Also, please be aware some crashes require additional investigation beyond the initial report. Additional information resulting from these investigations may not be available until such investigations are complete. If you have any questions about obtaining a crash report or are unable to view a report you have purchased, please send an e-mail to [email protected], or contact the Central Records Unit via mail, telephone or fax as indicated below.
Ordering Process
- Click the “Request an OSHP Crash Report/Photos Online” button at the top of this web page.
- Enter the crash report number and the year of the crash; OR, if you do not have the crash report number, enter the data into “Advanced Options”
- Enter the text from the image (reCAPTCHA)
- Click “Search”
Cost & Delivery
- Crash reports and photographs are $4.00 total for each report and are provided via web links. Crash reports and photographs purchased online will be available to you once your purchase is confirmed. The report/photographs will then be accessible by you for up to 72 hours for printing or downloading.
- Photographs that are ordered may be placed on a DVD; however, this will increase the cost from $4.00 to $12.00 (this includes the report and the DVD of photographs or just the DVD of photographs if purchased separately from the report). Crash photographs placed on a DVD will be sent in the mail. Please allow 4-6 weeks for delivery.
- Visa, MasterCard, Discover and American Express are currently accepted.
To order crash reports and/or photos online you must have a web browser capable of running Javascript, a valid credit card, and a viewer that is able to view PDF.

After the Purchase
Once the crash report has been purchased, and you find that you did not receive the entire investigation, you may e-mail [email protected] at a later date to see if any supplemental reports, such as witness statements, have been added. The supplemental reports will be e-mailed to you at no additional cost.
If you want to request audio/video records or reconstruction reports, please make a public records request by e-mailing [email protected] to specifically request those items. Be sure to include the crash number and the date of the crash in your request. If you are requesting a reconstruction report, please be sure that you have already purchased the crash report.

Complimentary Initial Report
You may obtain the initial crash report (OH-1 only) for free via the DPS Website by clicking on the following link: https://services.dps.ohio.gov/CrashRetrieval/OHCrashRetrieval.aspx. This website does not have the complete “official” report, however, the initial OH-1 report may be all you need for insurance purposes.
**Only fill in the date of crash, the last name of the driver, and the yellow text box, then click on “Get Report Data”. If you don’t know the last name of the driver, please only fill in the county, the date, and the yellow text box, then click on “Get Report Data”. Scroll down to view the available reports.
Public Records
The Ohio State Highway Patrol maintains criminal investigation reports taken by troopers and Ohio Investigative Unit agents. To order a criminal investigation report or to make a request for other public records under Ohio Revised Code section 149.43, please e-mail the Central Records Unit at [email protected].

As unfortunate as they are, accidents happen. So, you should be aware of what Ohio law requires you to do if you are in accident.
Accidents Happen
If you're driving a vehicle and are in an accident, you're required to stop your vehicle at once. You'll need to provide your name, address, and vehicle registration number to all the parties involved in the accident.
Additionally, if you're not the owner of the vehicle, you'll need to give the owner's name, too.
If it appears that no one has been hurt, you don't have to call the police. Be aware that police officers do not have to respond to calls concerning accidents that don't involve an injury.
If an injury has occurred, however, and is serious enough that the injured cannot either understand or write down the information you've given them, you must stay with the victim until the police arrive. (Unless, of course, you've also been injured, and are being removed from the accident scene in an emergency vehicle.)
Should an injury occur, or if there was at least $400 in property damage, and if the driver or owner of one of the vehicles in the accident doesn't have auto insurance or acceptable financial responsibility coverage, you'll need to file a Crash Report (Form BMV 3303) with the Bureau of Motor Vehicles (BMV).
Keep in mind you'll need to do so within six months of the accident date.
Accidents with Unoccupied Vehicles
It doesn't take two drivers to create an auto accident; sometimes, we do enough damage ourselves!
For all accidents involving an unoccupied vehicle, you're required to attach a note to the damaged vehicle complete with your name, address, and vehicle registration number. Make sure to write legibly, and affix the note to the vehicle in a secure, easy-to-see location.
Miscellaneous Information
If police arrive on the scene of an accident, they must fill out an accident report. A record of the accident will be listed on your driving record, even if you weren't at fault.
Should damage occur to someone's real or personal property, everyone involved in the accident must give the pertinent information to the owner (or person in charge) of the property.
If you are unable to do this at the time of the accident, you must relay this information to law enforcement officials that have jurisdiction over the property. You'll also need to describe what happened, and where the accident took place.
If you merely witnessed an accident but are not involved in it, you're not required to lend your assistance in any way.
Next of Kin Notification
The BMV now allows drivers to register their emergency contact information with the agency. By doing so, you enable law enforcement officials to more quickly notify a family member, guardian, or friend if you're seriously injured in an accident, or are unable to communicate. Register for this service online or at any license agency location.
